East Central Planning and Development District (ECPDD) is seeking a licensed social worker to work in the Elderly and Disabled Waiver Program. Excellent fringe benefits, no nights, weekends, or on-call. This position is involved in coordinating the Case Management Program, which consists of planning, arranging for and coordinating appropriate home and community-based services which should be provided to clients in a timely and cost-effective manner.
The person assigned to this position maintains considerable contact with service providers, clients, and works with a Registered Nurse case manager in the team approach to screen-referred clients and determines eligibility.
The pay range for the licensed Social Worker is from $40,000 to $45,000.

Responsibilities
- Supervises other social workers, therapists and interns.
- Supervises individual, group and family sessions for patients
- Member of the multi-disciplinary team; participates in chart reviews for quality assurance.
- Meets as required by state regulations with clinical staff to meet licensure or certification requirements
- Supervises the clinical department: discharge and after care planning, completion of discharge summaries, completion of clinical documentation and providing guidance to the clinical staff.
- Review psychosocial documentation, completion of outcome studies assessments and development of clinical notes.
- Establishes and maintains a supportive/directive leadership style to foster a positive organizational culture making self available to staff and possible providing LCSW supervision.
- Provides Clinical Social Work services: individual and family services including communication in an age/disability/cultural sensitive milieu.
- On-call weekends and holidays as needed. Major holidays off.
Requirements
- REQUIRED - LCSW or LMSW
- Must possess a Master’s degree in Social Work and prefer at least three years of psych experience
